INFORMATION FOR PATIENTS REFERRED BY THE NFZ

• Treatment begins on the date specified in the referral confirmed by the Fund. The first person-day begins at 2:00 PM on the first day of the stay, and the last person-day ends at 12:00 PM on the last day of the stay (in accordance with the Regulation of the Minister of Health of 28 August 2009 on guaranteed benefits in the field of spa treatment).

WHEN GOING FOR SPA TREATMENT, YOU SHOULD BRING

• the referral for spa treatment confirmed by the Regional Branch of the Fund
• proof of identity
• current proof of health insurance
• results of additional examinations, specialist consultations and discharge summaries from hospital treatment that may be related to the referral or treatment at the spa
• regularly taken medications for the entire duration of the stay

THE PATIENT BEARS THE FOLLOWING COSTS

• travel to and from the spa treatment
• partial co-payment for the costs of meals and accommodation in the spa sanatorium (in accordance with the Regulation of the Minister of Health of 28 August 2009 on guaranteed benefits in the field of spa treatment)
• the spa fee in accordance with the current resolution of the Ciechocinek City Council

ADDITIONAL INFORMATION

• Sundays and public holidays as well as arrival and departure days are days without treatments
• Additionally, you are obliged to pay the spa fee in accordance with the current resolution of the Ciechocinek City Council
